Mama Mentor LLC works with most major insurers. Families with plans that are in-network should incur no fees*. If we are not in-network with your plan, you may pay for services out-of-pocket with our discounted self-pay rates then file for out-of-network insurance reimbursement. We bill directly to in-network plans.
*Lactation services are covered under the Affordable Care Act (ACA). Per the ACA, new insurance plans are required by law to cover breastfeeding support & supplies in full. Copay/deductibles for lactation support should not apply but is dependent on your plan so we encourage you to call your plan to verify coverage prior to your visit. VA CCN:
We currently work with a third party billing company to help bill for VA CCN plans. You must obtain a referral through your maternity care coordinator prior to being seen. This referral must stay up to date for the duration of time you receive care. Please call us to discuss these plans further as the billing landscape has recently changed for some of these plans.
Medicaid:
We are not in-network with Medicaid plans at this time. You may choose to pay out-of-pocket at our discounted rates. You will be provided with a superbill to submit for reimbursement.
Self-pay:
We offer discounted self-pay rates if we are not currently working with your insurance plan. Consultation costs vary on time spent together as well as the complexity of the visit. Superbills are provided after any self-pay consultation so that you may seek insurance reimbursement.
PAYMENT IS DUE IN FULL AT THE TIME OF SERVICE IF YOU'RE SELF PAY. ALL MAJOR ELECTRONIC FORMS OF PAYMENT ACCEPTED INCLUDING CREDIT, DEBIT, HSA & FSA VIA SQUARE.
ADDITIONAL FEES APPLY FOR HOME VISITS & TRAVEL OUTSIDE OF A 15 MI RADIUS, AFTER-HOURS, WEEKENDS & HOLIDAYS. A FEE WILL APPLY FOR LATE CANCELLATION/RESCHEDULE WITH NO MORE THAN 24 TO 48 HOURS NOTICE.
