Insurance Coverage

Mama Mentor LLC works with most major insurers. Families with plans that are in-network should incur no fees*. If we are not in-network with your plan, you may pay for services out-of-pocket with our discounted self-pay rates then file for out-of-network insurance reimbursement. We bill directly to in-network plans.

*Lactation services are covered under the Affordable Care Act (ACA). Per the ACA, new insurance plans are required by law to cover breastfeeding support & supplies in full. Copay/deductibles for lactation support should not apply but we encourage you to call your plan to verify coverage prior to your visit. BCBS/Anthem/Cigna/Plans with Multiplan logo:

TLN is a third-party company we work with to help bill for most PPO plans including BCBS, Anthem, Multiplan, & Cigna. Lactation coverage approved through TLN is NO cost to you & provides an unlimited number of visits.

VA CCN:

We accept VA CCN plans through United Healthcare. You must obtain a referral through your maternity care coordinator prior to being seen. This referral must stay up to date for the duration of time you receive care. You may proceed to scheduling and enter your insurance information there for plan verification. Please provide a copy of your insurance card.

Tricare-Prime & Select:

We are in-network with Tricare East plans. You do not need to click the link below. You may proceed to scheduling and enter your insurance information there for plan verification. Please provide a copy of your Tricare card.

United Healthcare/UMR/GEHA (Georgia):

We are in-network with United Healthcare, UMR and GEHA GA plans. You do not need to click the link below. You may proceed to scheduling and enter your insurance information there for plan verification. Please provide a copy of your insurance card.

Aetna:

PLANS WE DO NOT WORK WITH yet

Medicaid:

We are not in-network with Medicaid plans at this time. You may choose to pay out-of-pocket at our discounted rates. You will be provided with a superbill to submit for reimbursement.

BCBS Federal:

We are not in-network with BCBS Federal. Unfortunately, these plans are considered grandfathered and typically do not cover lactation support. You may choose to pay out-of-pocket at our self-pay rates. You will be provided with a superbill to submit for reimbursement. We offer out-of-network billing at a nominal fee for your convenience.

Self-pay:

We offer discounted self-pay rates if we are not currently working with your insurance plan. Consultation costs vary on time spent together as well as the complexity of the visit. Superbills are provided after any self-pay consultation so that you may seek insurance reimbursement. We also offer filing on your behalf for a nominal fee so that you don't have to hassle with your insurer!
